As the Medtech industry is rapidly evolving, sponsors are progressively relying on computational modeling and simulation (CM&S) to speed up the development of new therapies by reducing reliance on bench, animal, and clinical testing.
However, as sponsors adopt CM&S technology, they are faced with a significant challenge: creating computational models that withstand the examination of regulators. As a result, sponsors are left wondering how they can embrace CM&S, which can drastically expedite the development of new therapies, while remaining compliant with FDA requirements.
Developing CM&S that satisfies regulatory standards remains a challenge for the industry for several reasons, including its inherent complexity and the associated regulatory requirements. For instance, the FDA has stated that demonstrating model credibility is a multifaceted process, encompassing verification, validation, uncertainty qualification, applicability analysis, and adequacy assessment. These requirements can pose a substantial challenge to meet repeatedly, stretching the capabilities of even the most advanced companies.
To address this obstacle, the American Society of Mechanical Engineers’ (ASME) Validation and Verification (V&V) 40 Committee in Computational Modeling of Medical Devices developed the ASME V&V 40 Standard in order to provide a framework to quantify the accuracy and credibility of CM&S that will be included in regulatory submissions. This standard is recognized by the FDA, which has generally been supportive of CM&S so long as it follows the modeling requirements. FDA guidance underscores that the growing significance of standards like V&V 40 will not only help to propel compliance but also to fuel further innovation of advanced models, purpose-built to develop new therapies faster and with patient outcomes in mind.
Specifically, the V&V 40 Standard is a risk-based standard that provides a structured framework to help bridge the gap between advanced computational techniques and strict regulatory requirements. The overarching objective is to have a standard in place that medical device manufacturers can easily reference to validate the credibility of their CM&S applications. Following a set framework can help ensure compliance to demonstrate the safety and efficacy of innovative therapies, while also reducing reliance on other forms of evidence.
While Class I medical devices are commonly considered to be quite low risk, they still require stringent quality assurance to guarantee the highest possible level of patient safety. The V&V 40 Standard plays an important role in this context for several reasons:

The V&V 40 Standard provides valuable guidelines for enhancing the credibility of CM&S, but it is not a regulatory requirement. However, V&V 40 does help developers better meet other regulatory requirements, which is particularly beneficial given the myriad of regulatory requirements that developers are expected to meet.
Among these requirements is the FDA’s Quality System Regulation under 21 CFR Part 820, which stipulates comprehensive requirements for the design, manufacturing, packaging, labeling, storage, installation, and servicing of medical devices. This regulation highlights the importance of maintaining a robust quality management system to ensure product safety and efficacy.

As Class I manufacturers embark on their development program, they should consider both the V&V 40 standard and mandated regulatory requirements to ensure compliance and accelerate development timelines.
The V&V 40 Standard represents a major advancement in the regulatory landscape for medical device development, particularly for Class I devices. By offering a standardized framework for validating and verifying CM&S, the standard is the best opportunity for developers to meet the quality, compliance and patient safety requirements that they are subject to.
While the V&V 40 can help to propel innovation, medical device manufacturers should also take into consideration the complexities and administrative burdens that are associated with implementing the V&V 40 framework to ensure that they are adequately prepared. By maintaining a proactive and informed stance, Class I medical device companies can not only achieve regulatory compliance, but also enhance the safety and efficacy of their products and accelerate development timelines to more quickly service patients.
